25 | EARLY AND MAINTAINED TREAT-TO-TARGET RESPONSE WITH IXEKIZUMAB UP TO 5 YEARS IN MODERATE-TO-SEVERE PSORIASIS: A POST-HOC ANALYSIS OF UNCOVER-3

Background. The National Psoriasis Foundation recommends evaluating treatment response at Week12, with a target body surface area (BSA) ≤1% affected and an acceptable response of BSA≤3%1. European consensuses emphasize a shift from relative outcomes such as Psoriasis Area and Severity Index (PASI) scores of 90/100 towards absolute endpoints including PASI≤22. Early achievement and long-term maintenance of these treat-to-target (T2T) outcomes are important, providing patients with more days of minimal disease burden.
Methods. UNCOVER-3 (NCT01646177) was a multicenter, randomized, double-blind, placebo-controlled study of ixekizumab in patients with moderate-to-severe psoriasis3. This post-hoc analysis examined T2T outcomes in patients from UNCOVER-3 receiving the label dose of ixekizumab (N=385). Baseline demographics and patient characteristics were similar to those previously presented4. Early and maintained response was defined as achievement of T2T goals at Week12 maintained at 5 years (Week264). Assessments included absolute PASI≤2, absolute PASI≤1, BSA≤3%, and BSA≤1%, reported as observed. No statistical testing for significance was conducted.
Results. At Week12, absolute PASI<2 and PASI<1 response rates were achieved by 71.7% (n=276/385) and 68.1% (n=262/385) of patients; among responders with available observed data at Week264, 94.5% (n=156/165) and 81.3% (n=126/155) maintained absolute PASI≤2 and PASI≤1 responses, respectively. Additionally, BSA<3% and BSA<1% response rates were achieved by 62.1% (n=239/385) and 50.9% (n=196/385) of patients at Week12; among these responders, 97.7% (n=128/131) and 88.7% (n=102/115) maintained these targets at Week264, respectively.
Conclusions. These findings demonstrate that early and continuous treatment with ixekizumab results in long-term improvements in skin clearance to 5 years across clinically important T2T outcomes consistent with guidelines recommendations.
